This chapter reviewed intensity-modulated radiotherapy (IMRT) development history from the early 1990s to the 2020s, highlighting how a new technique such as IMRT evolved during this period. The concept of IMRT originated from the design of missing tissue compensators. Advancements in computer technology, computed tomography images, computer-controlled multiple-leaf collimators, and computer treatment planning optimization enable clinical implementation of IMRT from this early phase of sequential tomotherapy to volumetric-modulated arc therapy. The success of IMRT heralds imaging-guided treatment delivery, thus improving treatment precision and accuracy. The synergy of IMRT and IGRT leads to stereotactic body radiotherapy with a short treatment course and potent treatment regimens.
