Figure 11.1 demonstrates the concept of sepsis shifted from SIRS to SOFA since SEPSIS-3 (2016). Definition of sepsis is NOT based on SIRS criteria (Table 11.1) but use q-SOFA (Table 11.2) or SOFA (Table 11.3) to assess patients. Follow Fig. 11.2 for diagnose patients with sepsis. Ruling out infection is not the goal of management of sepsis or septic shock patients, underlying inflammatory and immunological derangement encompasses many organs, that should be managed. Always prioritize delirium assessment, pain assessment and early mobilization, when patients come out of the MICU, provide SPA therapy (sleep, poop, and appetite).
