One of the impediments in liver transplantation has been the shortage of deceased donor organs and the technical difficulty encountered while transplanting pediatric patients. This led to the evolution of techniques using partial liver (PL) grafts such as split liver transplantation (SLT), reduced liver transplantation (RLT), and living donor liver transplantation (LDLT). Successful use of these grafts mandates a detailed understanding of liver anatomy. Meticulous donor evaluation and excellent surgical skills further help to minimize complications. Innovative techniques such as APOLT and evolving minimally invasive techniques such as laparoscopic and robotic donor hepatectomy and recipient transplantation have further revolutionized liver transplantation using partial grafts, with excellent short- and long-term outcomes. This chapter describes the liver anatomy, donor selection for retrieving partial grafts, and important technical concepts in LDLT and SLT, with a view to provide a better understanding of their clinical application.
