Effect of Radiofrequency Power on a Previously Desiccated Zone During Surgical Resection

Although radiofrequency (RF) power is commonly used in surgical resections to minimize blood loss by creating a coagulative necrosis in the tissue on the resection plane, there is still no information about whether applying RF on a severely desiccated (even charred) surface can create deep coagulation zones. To study this issue, a bench test based on excised liver was conducted with an internally cooled RF electrode. The results showed that coagulation zones can be created up to 3 mm below a previously desiccated surface. While an excessive power level causes smoke and a shallow coagulation, a medium level achieves the greatest coagulation depth. Monitoring the envelope of the RF current waveform, particularly its continuity, would provide information on the efficiency of the RF power application. Although the results suggest that coagulation can be created through a layer of highly desiccated tissue, the specific power level must be selected in the surgical practice, possibly using the evolution of the peak RF current value as the guiding parameter.
