This chapter assesses the impact of the changing attitudes to social and health care and the public and political discourse on reform which began to emerge in Britain at the turn of the twentieth century. With the LGB and its Medical Department both diminished by decades of under-resourcing and increasingly left rudderless by their tired leadership, it considers the apparent stagnation in thinking and the consequences of their failure to recognise the tide of change or engage in the dialogue.
