Professional Voice and Muscle Tension Dysphonia: The Art of the Multi-disciplinary Team

This chapter examines Muscle Tension Dysphonia (MTD) in professional singers, a functional voice disorder characterized by excessive laryngeal muscle tension that can significantly impact vocal performance and career sustainability. Professional singers face unique vocal demands requiring extraordinary coordination and endurance, making them particularly vulnerable to MTD’s effects on voice quality, range, and stamina. The chapter distinguishes between primary MTD (functional muscle hyperactivity without structural abnormalities) and secondary MTD (compensatory tension due to underlying pathology). Clinical presentations include vocal fatigue, decreased range, projection difficulties, and throat constriction sensations. Central to effective management is the multidisciplinary approach combining laryngologists and voice-specialized speech-language pathologists (SLPs). Laryngologists provide medical evaluation through videostroboscopy, while SLPs deliver evidence-based voice therapy including Semi-Occluded Vocal Tract Exercises, Resonant Voice Therapy, and laryngeal massage techniques. The chapter emphasizes Self-Determination Theory principles in therapy, empowering patient autonomy and motivation. For treatment-resistant cases, emerging interventions include botulinum toxin injections targeting false vocal folds, minimizing vocal quality risks. This comprehensive resource demonstrates how collaborative expertise between medical and speech-language pathology creates optimal outcomes for professional singers with MTD.
