It was time for hemostasis and closing in a completely uneventful liver transplantation. There were some bleeding varices under the diaphragm that needed attention. While the surgeons began to close with prolene sutures, the blood pressures became soft and minutes later, fell to a catastrophic 40/20. The surgeons reopened and could not find any bleeding in the abdomen, including the retroperitoneal space. The hematocrit prior to closing was thirty, and during the search for bleeding, the hematocrit fell to 16. After a transfusion and high-dose pressors, the patient had a cardiac arrest.
