Portable Noninvasive Ventilation Devices
摘要
Noninvasive ventilation (NIV) has shown significant benefits over standard oxygen therapy, improving dyspnea, physical activity, and health-related quality of life while reducing hospital admissions for COPD exacerbations. Portable NIV devices, such as the Philips Respironics Trilogy Evo and ResMed Astral 100, have advanced to enhance mobility despite limitations in motion due to device size. The VitaBreath Philips Respironics offers a lightweight, wearable solution with CPAP and PSV modes, auto-titration, and a ramp function for comfort. Negative-pressure ventilation (NPV) represents a physiological alternative, historically prominent during the polio pandemic and revisited during the SARS-CoV-2 pandemic. Devices like the iron lung, Exovent, and vest ventilators provide options for patients unable to tolerate facial interfaces or requiring unique support. Modern innovations, including the cuirass ventilator and intermittent abdominal pressure ventilator, offer expanded utility. NPV advantages include reduced airway injury risks and hemodynamic stability, though they are constrained by mobility challenges and less effectiveness in patients with low lung compliance.
