Upper gastrointestinal endoscopy is considered the best technique for visualizing the mucosa of the upper gastrointestinal tract. One of the main contraindications to endoscopy is acute respiratory failure, so a patient with this condition should be stabilized before proceeding with endoscopy. Sedation and general anesthesia increase the risk of respiratory failure and pose life-threatening risks. Noninvasive ventilation (NIV) can be used to prevent respiratory failure in high-risk patients, including obese patients at risk of hypoventilation during sedation and patients with a history of respiratory disease. Special NIV masks have been developed that are shaped to tightly cover the face, nose, and mouth, providing a smooth and unrestricted flow of oxygen as air is inhaled and exhaled from the airway through a one-way valve. In addition, the high-flow nasal cannula (HFNC), which delivers oxygen, is one of the possible techniques used in gastrointestinal endoscopy. NIV and HFNC allow necessary endoscopic procedures to be performed safely in patients with respiratory failure without the need to intubate them.
