Applications of NIV in Patients Undergoing Coronary Angiography

Patients with acute coronary syndromes may present with respiratory failure due to acute pulmonary edema. This is a life-threatening condition that requires multifactorial treatment. At the same time, immediate coronary angiography and angioplasty of the infarct-related artery are indicated. Noninvasive ventilation (NIV) has been shown to reduce mortality, need for intubation, and decrease dyspnea in patients with acute pulmonary edema. It can expedite coronary angiography and therefore shorten door-to-balloon time. CPAP is the preferred ventilation mode for treatment of respiratory failure due to pulmonary edema. Basic initial settings include FiO2 of 100% and PEEP of 5 cm of water. Apnea support and minute ventilation alarm should also be programmed. Treatment should be initiated as soon as possible, preferably in the prehospital setting.
