Wrong-site nerve blocks are preventable adverse events in regional anaesthesia that can lead to delays in surgery, patient harm, and medicolegal consequences. The estimated incidence is 1 in 6250, significantly higher than wrong-site surgery. Risk factors include time pressure, patient positioning changes, distractions, communication failures, and inadequate surgical markings. Prevention strategies involve pre-procedural checklists, clear site markings, standardized timeouts, team communication, and adherence to initiatives like “Stop Before You Block”. Consequences include surgical delays, local anaesthetic toxicity risks, patient dissatisfaction, and compromised safety. A structured, team-based approach is essential to eliminate preventable errors and enhance patient safety in regional anaesthesia.
