Inguinal hernia repair is associated with postoperative pain. The cutaneous nerve supply is derived from the descending branches of the thoracolumbar spinal nerves, such as the ilioinguinal nerve and iliohypogastric nerve (T12, L1). Various local, general, and regional anaesthesia techniques have been described for hernia repair depending on institutional protocols and expertise of anesthesiologists. The regional blocks become more important in patients with comorbidities like chronic obstructive pulmonary disease due to increased risk involved with giving general anesthesia. VArious choice of regional anesthesia techniques include wound infiltration by the surgeon, field block, ilioinguinal and iliohypogastric nerve block along with spermatic cord or Genitofemoral nerve (GFN) block, neuraxial anaesthesia, paravertebral block, quadratus lumborum block, anterior TAP block and fascia transversalis block. The PROSPECT guidelines suggest a combination of nerve blocks and drugs for an effective analgesia.
