Liver resection remains the mainstay surgical treatment for most benign or malignant hepatic neoplasms when feasible. The right posterior sectionectomy (RPS) is a technically challenging operation performed for posterior right lobe lesions. The central hepatectomy (CH) that includes removal of segments 4, 5, 8 and the right anterior sectionectomy (RAS) that includes removal of only segments 5, 8 are similarly challenging operations undertaken for central lesions. These approaches allow preservation of liver volumes while maintaining the principle of anatomic resection. They have demonstrated safety and efficacy when approached either open or through minimally invasive techniques. This chapter addresses indications and preoperative planning for RPS, RAS, and CH, and technical approach.
