The minimization of intraoperative bleeding has historically been a major concern in liver surgery, with continuous efforts focused on optimizing intraoperative and postoperative outcomes through technical advancements. Acquiring safe and adequate peri-procedural vascular control is therefore crucial. Over the last few decades, as liver resection transitioned from open to minimally invasive procedures globally, various techniques for accessing, dissecting, and controlling structures within the hepatic hilum and its deeper branches have been meticulously described. These approaches generally fall into three main categories: the intrafascial, the extrafascial, and the extrafascial transfissural approach. Numerous studies have investigated the advantages and potential drawbacks of each approach compared to others available, both in open and laparoscopic settings. However, robust evidence regarding their ideal application in the most modern contexts such as robot-assisted liver surgery remains scarce.
