However, in the 1970s, Yasargil published a detailed description of the approach, emphasizing the exposure of the Sylvian fissure at the apex of a triangular region between the frontal and temporal lobes while minimizing brain retraction. It is important to note that a significant amount of brain retraction during a neurosurgical procedure occurs not just during deep dissection but also at the stage of craniotomy, as the brain is exposed and access to the target region is established. By refining the pterional craniotomy, access to the Sylvian fissure was significantly improved, along with exposure to natural cisternal spaces, such as the opticocarotid and oculomotor cisterns, which can be reached directly through the Sylvian fissure [1, 2].
