The subtemporal approach is a surgical technique used in neurosurgery to access lesions in the middle cranial fossa, upper brainstem, and posterior circulation. It can be performed extradurally, where the temporal dura is elevated to expose the middle fossa structures, often used for petroclival lesions and cavernous sinus pathology, or intradurally, where the dura is opened to access the mesial temporal lobe, cranial nerves III and IV, basilar tip, superior cerebellar artery, and ambient cistern [1–4].
