Coordinating holistic palliative care for older people across different settings (home, hospital, hospice) is challenging due to complex needs and non-interoperable healthcare systems. AgeTech provides key solutions for improving care coordination. Technologies, such as smartwatches, smart home monitoring systems (e.g., Howz), and remote consultation platforms (e.g., Accurx) facilitate early diagnosis, real-time remote patient monitoring, and efficient communication between providers. Initiatives like EPaCCS and the Serious Illness Care Program illustrate how shared electronic records and systematic conversation guides enhance timely support and ensure care aligns with patient preferences, especially for frail individuals nearing the end-of-life.
