Total joint replacement (TJR) of the temporomandibular joint (TMJ) is a critical surgical intervention for patients with severe TMJ pathology, including ankylosis, degenerative diseases, neoplasia, and failed previous reconstructions. Virtual Surgical Planning (VSP) has revolutionized the precision, predictability, and efficiency of TMJ TJR by integrating patient-specific imaging, digital modeling, and custom implant fabrication. This paper provides a step-by-step guide to VSP for TMJ TJR, detailing the indications for joint replacement, imaging protocols, surgical planning considerations, and the integration of custom alloplastic implants. Key elements include the assessment of skeletal discrepancies, occlusal plane modifications, and aesthetic considerations. The process of virtual planning—conducted in collaboration with engineers and surgeons—is outlined, emphasizing the importance of occlusal analysis, midline correction, and proper condylectomy planning. Additionally, challenges such as nerve and vascular injury, implant adaptation, and potential complications are discussed, along with strategies for prevention and management.
