Distal radius malunion is the most common complication after distal radius fractures. Management of malunion requires distal radius osteotomy to restore anatomical relationship of the distal radius and the radiocarpal joint. The use of personalized custom drilling and cutting guides have gained popularity and the use of virtual surgical planning in the management of this complex problem is now commonplace. Here, we highlight a case of an untreated distal radius fracture, requiring corrective osteotomy of the radius and ulnar shortening using virtual surgical planning.
