Imaging has become a ubiquitous and central component of the clinical care of patients with respiratory disease, and is increasingly used to evaluate the pulmonary vasculature. In this chapter, we review the role of imaging modalities in pulmonary hypertension associated with chronic lung disease, including applications for screening, diagnosis, sub-phenotyping, and monitoring of disease response. We examine the strengths and limitations of existing imaging metrics, including those derived from CT, echocardiography, and MRI, that are currently used to identify the presence of PH in the setting of various lung diseases such as COPD and ILD. We also review the promise and the challenges of integrating existing imaging metrics into clinical prediction tools. Finally, we provide an overview of emerging imaging technologies for assessing the pulmonary vessels, and their potential applications to affect the care of patients with lung disease and pulmonary hypertension.
