The scapular tip and lateral scapular border free flaps can be harvested as chimeric free flaps including muscle from the latissimus dorsi or serratus anterior muscles, as well as skin over the parascapular and scapular regions. The vascular supply is from the subscapular artery and vein. The scapular tip is supplied by the angular artery, while the lateral scapular border is supplied by the circumflex scapular artery, which allows for multiple independently vascularized bone segments. Advantages of the scapular flaps include its chimeric nature with mobility of soft tissue components relative to bone, reliable vascular anatomy with good caliber vessels and lack of atherosclerosis, acceptable donor site morbidity, and the ability to harvest in a two-team approach.
