The deltopectoral flap was first reported by Aymard in 1917 and popularized by Bakamjian in 1965. The arterial supply is via the first three perforators from the internal mammary artery and drainage is via venae comitantes and the internal mammary vein. The deltopectoral flap can be utilized as a two-staged procedure with ultimate division of the pedicle or a single-staged island flap. The deltopectoral flap is less commonly used in recent times due to the risk of distal tip necrosis, the popularization of other more reliable regional flaps, and the increased utilization of microvascular free flap reconstruction. Nonetheless, it remains a robust and reliable reconstruction option for select head and neck defects.
