In this chapter, the aneurysms of the cerebellopontine angle arteries and the aneurysms that can be surgically treated through the retrosigmoid and far-lateral approaches are reviewed. The vascular anatomy of the cerebellopontine angle arteries is reviewed in detail. Cerebellopontine angle aneurysms represent a subtype of posterior circulation aneurysms, arising from the arteries of the posterior circulation as they traverse the cerebellopontine angle. The vertebral artery and basilar artery trunks, perforators, and the vertebral artery and posteroinferior cerebellar artery junction are common locations for the formation of saccular aneurysms. Dissection occurs frequently in both the basilar artery and the vertebral artery. The vertebral artery and the basilar artery are implicated in fusiform aneurysms. Microneurosurgical clipping is recommended for saccular aneurysms located at the vertebral artery and the posterior inferior cerebellar junction. There are precise indications for other types and locations. Endovascular therapy represents an alternative approach.
