Congenitally corrected transposition of the great arteries (ccTGA) is a complex congenital heart defect that is defined by reversed (discordant) atrioventricular (AV) and ventriculoarterial (VA) connections. Physiologic (leaving the right ventricle in the systemic circuit and the left ventricle in the pulmonary circuit) and anatomic (placing the right ventricle in the pulmonary circuit and the left ventricle in the systemic circuit) repairs have been used. This chapter will discuss the multiple operations for ccTGA including anatomic correction via the double switch operation, Senning or Mustard, and intra-atrial baffle plus translocation of the great vessels. In addition, we will examine the roles of the Fontan operation and cardiac transplantation for complex ccTGA. The attendant complications related to these procedures will be explored.
