The four anatomic features that characterize Tetralogy of Fallot (TOF) are: (1) stenosis of the pulmonary artery, (2) ventricular septal defect (VSD), (3) rightward deviation of the aortic root, and (4) right ventricular hypertrophy. Surgical measures to treat TOF have not been settled and questions still remain regarding timing of the intervention, pre-repair palliation, and valve preservation options. In this chapter complications associated with palliative procedures, right ventricular outflow tract obstruction, direct closure of the VSD, tricuspid disruption, aortic valve injury, chordal transection, and injury to the leaflets will be discussed and methods to resolve these complications will be addressed.
