Risks of Revascularization: The So-Called Treatment Trap

Despite extensive evidence and practice guidelines endorsing a conservative approach to claudication, revascularization is commonly performed in the United States. Given the limited durability of infrainguinal interventions, next steps in management when a prior stent and/or bypass occludes and symptoms return is a common challenge in routine practice. The concept of a treatment trap is described, whereby patients may incur repeated interventions, with increasing complexity and risk. Given the benign limb prognosis in cases of conservative management, this treatment trap highlights the need for very thoughtful consideration of risks and benefits before undertaking an initial revascularization, with a focus specifically on the longer term when durability rates are known to diminish.
