This study explores the co-design, development, and implementation of an accessible Personal Plan with an integrated health records dataset within a cloud-based system to enhance service delivery for adults with intellectual disabilities in Ireland. Paper-based planning systems created fragmentation, duplicated effort, and inconsistencies in data integrity. Using Action Design Research (ADR), the project engaged service users, families, frontline staff, and regulators in iterative co-design to build a standardised digital dataset integrated into the iplanit platform. Preliminary results show improvements in record-keeping, real-time access, data coordination, and regulatory compliance. This paper presents learnings from the pilot phase, highlights opportunities for system-wide scaling, and reflects on co-design practices as a model for inclusive digital transformation.
