Control of hemorrhage is critical for stabilization of patients presenting with traumatic injuries, particularly those that are abdominopelvic in location. Working in an interdisciplinary fashion with trauma and critical care surgical teams, the field of interventional radiology provides a pillar of clinical care which is invaluable to the stabilization effort. Through an image-guided minimally invasive approach, which includes endovascular and transcutaneous therapies, definitive control of the bleeding source is offered, without the physiological toll associated with open surgery. Embolization is the primary technique that is employed to promote cessation of bleeding at the site of acute arterial injury, which may present as a transection, pseudoaneurysm, or arteriovenous fistula. Both temporary and permanent embolization options are available depending on the clinical scenario and vascular anatomic site in question. The grade and degree of organ injury as well as the patient’s hemodynamic status may in turn guide the specific intervention selected. Integration of the interventional radiology team in traumatic injury care algorithms to support patients with suspected or demonstrated active hemorrhage allows for early multidisciplinary decision-making and the promotion of optimized patient outcomes.
