A prevalent liver condition known as nonalcoholic fatty liver disease (NAFLD) is a leading cause of cryptogenic cirrhosis and chronic liver disease, particularly in developed countries. NAFLD pathogenesis is multifactorial and involves critical processes such as resistance to insulin, lipid metabolism disruption, oxidative stress, inflammation, programmed cell death, and fibrosis. Collectively, these processes contribute to disease progression. The transition from simple steatosis (fatty liver) to nonalcoholic steatohepatitis (NASH) is often driven by inflammation triggered by lipid accumulation. Therefore, the effective amelioration of steatosis and inflammation is crucial for successful NAFLD therapy. In recent years, herbal medicines have garnered significant interest as potential treatments for NAFLD, primarily because of their wide availability, fewer side effects, and documented therapeutic mechanisms. Various herbal derivatives that have shown promise in targeting the underlying pathological mechanisms of NAFLD have been extensively studied. Garlic, milk thistle, red grapes, and ginger are widely used in the treatment of NAFLD. The combination of such botanicals has already been employed in the Ayurvedic system, and research has indicated that herbal medicines can improve steatosis and inflammation through multiple mechanisms that influence lipid metabolism and inflammatory pathways. These herbal treatments have been evaluated in preclinical studies and clinical trials and have shown beneficial effects on liver health without significant adverse effects. This chapter highlights the protective effects of various herbs in their natural or medicinal forms and their mechanisms of action in ameliorating steatosis and inflammation. This emphasizes the importance of traditional herbal preparations, crude extracts from medicinal plants, and pure natural compounds as valuable resources for groundbreaking drug research and future advancement of NAFLD treatments.
