Nowadays, injectable fillers are a common component of face rejuvenation procedures. Thanks to the development of several long-lasting and biocompatible filler materials, especially hyaluronic acid (HA) fillers, many unfulfilled demands can now be handled in a repeatable and predictable way, contributing to their rising popularity. Most of these procedures treat aging-related volume loss in the face, which can lead to several changes early and late in life. These no-downtime treatments are preferred in most therapeutic situations due to their immediacy, predictability, and safety. The aging face can be more effectively corrected with the addition of volume or shape restoration, energy-based treatments (such as radiofrequency or lasers) for changes to the skin’s surface (such as wrinkles or pigmentation), and surgical lifting for laxity of the skin. No filler is ideal for every situation, as previously mentioned; for example, picking out a golf club for a particular shot, certain clubs may be playable by an expert. Of course, there are times when specific options would not cut it; for instance, you cannot expect a soft, spreadable filler to give your cheeks a defined contour. In this author’s hands, the expected depth of placement is the starting point for filler choices. Weight, in the form of increased viscosity and cohesivity, is a common characteristic of heavier subcutaneous or subperiosteal (orbital) fillers. This filler class includes high-viscosity, high-cohesivity HA, and calcium hydroxylapatite (CaHa). Deep dermal tissue planes and the dermal/subcutaneous junction are ideal for initial, lower-viscosity HA fillers.
