Weaning neurological patients requires some special considerations in addition to those already known for weaning patients in the general population. Many neurological patients can tolerate a spontaneous breathing trial but cannot tolerate extubation due to an inability to protect the airway. Weaning indices may not be accurate in this population because the reason for initiating mechanical ventilation is often related to decreased level of consciousness. These and other special considerations are discussed in this chapter.
