Single Anastomosis Duodeno-Ileal Gastric Bypass (SADI) has emerged as a popular bariatric and metabolic surgery procedure, particularly following its endorsement by the American Society for Metabolic and Bariatric Surgery (ASMBS) in 2021. While traditionally performed laparoscopically, the robotic approach offers several advantages, including enhanced visualization, precision, and dexterity. This chapter provides a detailed technical description of the robotic SADI procedure, emphasizing key steps such as duodenal dissection, window creation, stapling, and anastomosis. The importance of Indocyanine Green (ICG) fluorescence imaging for real-time assessment of duodenal perfusion is highlighted. By mastering the robotic SADI technique, bariatric surgeons can offer patients a safe and effective treatment option for weight loss and metabolic disease.
