Gastroesophageal reflux disease (GERD) is a prevalent condition in pediatric patients, particularly in neurologically impaired children, often necessitating surgical intervention when medical therapy fails. Robotic-assisted gastric fundoplication offers a minimally invasive alternative to traditional laparoscopic approaches, potentially enhancing precision, dexterity, and visualization in pediatric surgery. This chapter presents a comprehensive guide to the robotic-assisted Nissen and Toupet fundoplication procedures using the Da Vinci Xi system. Key aspects include preoperative diagnostics and planning, patient positioning, step-by-step port placement, robotic docking, dissection techniques, and wrap construction. Emphasis is placed on critical technical elements such as liver retraction, safe dissection near vital structures, proper creation of the retroesophageal space, and meticulous suturing to avoid complications like esophageal narrowing or wrap migration. Despite longer operative times compared to conventional laparoscopy, robotic-assisted techniques have proven to be safe and effective in pediatric cohorts. Postoperative recovery is generally swift, with early reintroduction of oral feeding. This chapter underscores the importance of surgeon experience and standardized techniques in achieving optimal outcomes and positions robotic-assisted fundoplication as a valuable tool in the surgical treatment of pediatric GERD.
