Anaphylaxis is a severe, systemic allergic reaction triggered by prior sensitization to an allergen, leading to IgE-mediated mast cell degranulation and the release of vasoactive mediators. This results in widespread vasodilation, increased capillary permeability, and fluid shifts, leading to hypotension, edema, and respiratory distress. In this case, a 45-year-old botanist presented to the emergency department with a diffuse skin rash, dyspnea, cyanosis, and an audible wheeze, indicative of an anaphylactic reaction following a bee sting, triggering profound cardiovascular collapse and myocardial ischemia. Despite initial treatment with vasopressors and inotropes, she required an intra-aortic balloon pump for hemodynamic stabilization. This case reviews important physiological concepts of fluid balance and blood pressure regulation. Additionally, the case underscores the importance of early recognition of anaphylaxis, appropriate emergency interventions including epinephrine administration, and long-term management strategies such as patient education on allergen avoidance and epinephrine auto-injector use to prevent future fatal episodes.
