With the increasing success of solid organ transplantation, solid organ transplant recipients are experiencing an increased life expectancy and thus may develop longer-term metabolic complications. These complications include cardiovascular disease, hypertension, hyperlipidemia, diabetes, chronic kidney disease, metabolic dysfunction-associated steatotic liver disease (MASLD), gout, and osteoporosis. Risks for these conditions include both transplant-specific factors such as the side effects of immunosuppressive medication as well as medical comorbidities, lifestyle, and other factors. This chapter reviews the epidemiology, pathophysiology, screening, prevention and treatment of common metabolic complications affecting solid organ transplant recipients.
