Treatment of Fibromyalgia

Fibromyalgia (FM) is a complex, chronic pain disorder that often requires a multidisciplinary treatment approach. Non-pharmacologic strategies such as patient education, cognitive behavioral therapy (CBT), mindfulness-meditation, exercise, dietary modifications, and acupuncture play a critical role in symptom management. These interventions help reduce pain, improve the psychological well-being, and enhance overall quality of life. Pharmacologic treatment is also needed in the majority of the cases. The FDA-approved medications are duloxetine, milnacipran, and pregabalin. Other non-FDA approved medications for FM treatment include amitriptyline, fluoxetine, cyclobenzaprine, and tramadol. Treatment adherence remains a challenge due to side effects and the heterogeneous nature of FM. While these medications can alleviate pain and associated symptoms, side effects such as xerostomia, nausea, and reflux may contribute to dysphonia and voice dysfunction in FM patients. Future research should focus on optimizing individualized treatment plans through pharmacogenomics. Combining pharmacologic and non-pharmacologic therapies to improve long-term outcomes is recommended at present but also needs further study.
