Lymphadenectomy (LND) in renal cell carcinoma (RCC) is a highly debated topic in urological oncology. While its role as the gold standard for staging lymphatic spread is undisputed, its therapeutic potential remains controversial. Some argue that LND is primarily a diagnostic tool, while others believe it could improve survival in carefully selected cases. This chapter contrasts findings from the EORTC 30881 trial, which showed no survival benefit of LND in node-negative RCC, with retrospective studies suggesting potential therapeutic value for high-risk patients. It highlights the challenges posed by the unpredictable anatomy of renal lymphatic drainage and the limitations of current imaging in detecting micrometastases, reinforcing LND’s importance for accurate staging. Importantly, precise staging could enable access to advanced treatments, including novel systemic therapies that align with disease severity. Advances in imaging, molecular profiling, and biomarker research offer the promise of more refined patient selection for LND. As the debate continues, redefining lymph node involvement as stage IV disease could transform RCC management and prognosis. The future of LND lies in personalized medicine, supported by robust clinical trials and the integration of innovative treatment options.
