Anthracyclines play an important role in the treatment of cancer; however, accompanied cardiotoxic side effects are of major concern. A significant subset of patients treated with anthracyclines may develop left ventricular dysfunction, arrhythmias, and heart failure. Mechanistically, oxidative stress, direct myocardial injury, and dysregulation of cellular signaling are altered by anthracyclines. To identify high-risk patients, efforts have been focused on elucidating risk factors for anthracycline-induced cardiotoxicity (AIC). Both modifiable and nonmodifiable risk factors include smoking history, alcohol use, obesity, sex, race, and prior cardiovascular history. In this chapter, we focused on outlining current evidence for AIC risk factors prior to anthracycline treatment.
