The pathologic diagnosis and understanding of liver diseases are deeply rooted in morphology. As a result, analyzing liver biopsy specimens under a microscope and interpreting the morphologic changes play a central role in diagnosing and managing patients with liver disease. While advancements in imaging techniques, the rise of noninvasive blood-based tests, and the growth of artificial intelligence have led some to question the need for liver biopsy, it remains a critical tool for accurately diagnosing both benign and malignant liver conditions. Beyond diagnosis, the tissue obtained from liver biopsy can provide an abundance of information, allowing for improved treatment guidance, prognostication, targeted therapies, molecular characterization, and biomarker identification. Therefore, liver biopsy remains a mainstay in the workup and management of liver disease. This chapter provides a comprehensive guide on liver biopsy evaluation, covering both nonneoplastic and neoplastic specimens. The utilization of ancillary studies, such as immunohistochemistry, special stains, and electron microscopy, in liver biopsy assessment will be discussed. Additionally, this chapter will address recent advancements in molecular and genomic testing, reflecting their growing role in contemporary hepatopathology practice.
