PCI for CTO is one of the most challenging endovascular procedures performed today. Success rates for CTO PCI have improved significantly in recent years due to increased operator experience, advancements in CTO-specific equipment and algorithms, and the publication of expert consensus documents. This chapter aims to summarize the current evidence for CTO PCI, discuss success and risk prediction scoring systems, outline procedural principles and challenges, and provide insights into the future role of CTO PCI.
