Hydatid disease is a parasitic disease that is distributed globally. Up to 75% of all hydatid cysts are found in the liver. Patients may be asymptomatic for years and usually present with nonspecific complaints. However, at any time any particular hydatid cyst can develop complications which can be life threatening unless treated early. Complications are seen in about 40% of patients with hepatic hydatid disease. In some cases, complication may be the first manifestation of underlying hepatic hydatid disease while other complications may develop in patients being treated medically or patients being prepared for operative intervention after diagnosis. The most common complication of liver hydatid disease is superimposed infection, followed by rupture to the biliary tree, rupture to the peritoneal cavity, and rupture to the pleural cavity or the bronchus. Surgical treatment is the mainstay of treatment of complicated hydatid cysts.
