A 43-year-old male patient with a history of remote inferior myocardial infarction treated with PTCA and stent implantation at the level of his right coronary artery at the age of 34 years and ischemic cardiomyopathy with moderate LV dysfunction (LVEF of 44%) is admitted to the Cardiology Department for an episode of palpitations accompanied by chest pain, dyspnea, anxiety, and lightheadedness that had started 40 min prior to his presentation to the hospital. His ECG recorded at admittance to the Cardiology Department is presented in Fig. 1.
