Chronic heart failure (CHF) presents complex challenges that demand a multidisciplinary approach for effective management. This chapter investigates into the collaborative efforts of cardiovascular professionals, including cardiologists, cardiac nurses, rehabilitation specialists, dietitians, and psychologists, in delivering comprehensive care for CHF patients. Drawing upon European and American guidelines, it explores the integration of patient-centered care, contemporary healthcare trends, and evidence-based interventions within a multidisciplinary framework. Effective management of heart failure patients requires a comprehensive and coordinated approach, involving a multidisciplinary team of healthcare professionals.
