Diabetic Foot Osteomyelitis (DFO)

Diabetic foot osteomyelitis (DFO) is a severe complication of diabetes mellitus, generally categorized as acute or chronic, based on the histopathological findings or the duration of the symptoms and the infection. It is characterized by bone infections often resulting from diabetic foot ulcers. Affecting 20–70% of diabetic ulcers, DFO often leads to amputation, imposing significant physical, emotional, and financial burdens on patients and healthcare systems. With the increasing prevalence of diabetes, managing DFO requires effective collaboration among multidisciplinary medical teams. Diagnosis is complex, relying on clinical signs, imaging, and biopsies. An inflammatory toe (“sausage toe”), chronic ulcers, positive probe-to-bone tests, and radiographic evidence of bone destruction are key indicators. Imaging techniques like X-ray, MRI, CT, and PET-CT are crucial, while biomarkers (ESR, CRP) aid assessment. Histological analysis of bone biopsies remains the gold standard for diagnosis, also challenging in some cases. Treatment has shifted from radical amputations to conservative methods. Prolonged antibiotic therapy, often combined with surgical debridement, aims to preserve functional tissue. Antibiotics with high bone penetration are administered intravenously for 6–8 weeks, though optimal duration is debated. Minimally invasive surgeries have shown promising results in managing Charcot foot, chronic cases, and reducing amputations. Challenges include antibiotic resistance, delayed diagnoses, and high recurrence rates. Preventive strategies, including patient education, glycaemic control, and early detection of ulcers, are crucial to mitigating DFO risks. Emerging therapies (hyperbaric oxygen therapy and VAC-therapy) improve outcomes in refractory cases. Managing DFO effectively requires individualized approaches, considering factors like vascular health and infection severity. Despite advances, improving diagnostic accuracy and therapeutic efficacy remains essential in addressing this debilitating condition.
