Management of Bleeding in Antiplatelet-Treated Patients

Bleeding is a potentially life-threatening complication associated with antiplatelet therapy, requiring careful evaluation and management to balance the risks of hemorrhage and thrombosis. This chapter discusses the general principles for reversing the effects of antiplatelet agents, including the use of platelet transfusions, monoclonal antibodies (bentracimab), desmopressin, recombinant activated factor VII, tranexamic acid, and adsorption devices. Specific clinical scenarios such as gastrointestinal bleeding, intracranial hemorrhage, and trauma are reviewed with evidence-based recommendations. The perioperative management of antiplatelet agents in surgical patients is also addressed, including indications for intravenous bridging therapies. While current treatment options aim to mitigate bleeding risk, challenges remain in optimizing reversal strategies without exacerbating thrombotic complications. Advancements in diagnostic tools, refinement of risk stratification models, and the development of novel targeted reversal agents will be crucial in guiding therapeutic decisions and ensuring both efficacy and patient safety.
