Cognitive impairment in multiple sclerosis (MS) is one of the most common symptoms, significantly worsening the quality of life. The etiology of CI in MS remains unknown, but the strong correlations with neurodegeneration (brain atrophy) are recognized. This study explores the relationship between cognitive performance and brain morphological and functional changes in patients with multiple sclerosis. Altogether, 35 relapsing-remitting patients with MS with cognitive evaluation by Montreal Cognitive Assessment (MoCA) underwent resting-state electroencephalography (EEG) and magnetic resonance imaging (MRI) of the head. Positive correlations were found between MoCA scores and the volume of the left thalamus, putamen, hippocampus, amygdala, hemisphere cortex, subcortical gray matter, and cerebral white matter. The decline in the MoCA score was correlated with an increase in absolute power in the gamma band in frequency analysis, a decrease in intrahemispheric coherence in theta band, and an increase in interhemispheric phase lag index in the alpha and beta bands. These findings suggest that structural and functional brain changes are linked to cognitive impairment in MS, underscoring the value of neuroimaging and bioelectrical biomarkers.
