Pressure-Induced Stromal Keratitis and Persistent Epithelial Defect (PED) Masquerade Syndrome

Pressure-induced stromal keratitis is a laser-assisted in situ keratomileusis (LASIK) complication that mimics diffuse lamellar keratitis. It represents a pocket of fluid in the lamellar interface due to elevated intra-ocular pressure (IOP), usually steroid induced. This is frequently missed by the examiner due to a falsely measured low or normal IOP. Treatment includes cessation of topical steroids and lowering of the IOP pharmacologically. Persistent epithelial defect (PED) masquerade syndrome may develop at the edge of a LASIK flap with underlying stromal edema associated with epithelial ingrowth into the interface. Treatment includes flap lift, removal of the ingrown epithelium, and frequently suturing the flap with 10-0 nylon suture.
