Vesicoureteral reflux (VUR) can cause congenital or acquired renal nephropathy. Both forms predispose their carriers to renal damage ranging from mild hypertension to end stage renal disease in the most severe cases. Congenital VUR nephropathy is characterized by kidney dysplasia thought to be a result of high pressure on the developing kidney while acquired VUR nephropathy is characterized by irreversible renal scarring facilitated by recurring insults to the renal parenchyma by bacterial infection. The mechanisms for the development of parenchymal damage in both VUR forms are complex. The following chapter will try to summarize what is known so far about the elaborate physiological processes behind both entities.
