Electronic Health Record (EHR) data harbors many patient-specific information, comprising structured data like diagnoses, medications, and laboratory results, alongside unstructured data from clinical reports. The advent of extensive computational analyses utilizing patient observational data has catalyzed collaborative efforts among multiple healthcare institutions. However, efficient data amalgamation from disparate sources poses challenges due to interoperability issues. The Observational Health Data Sciences and Informatics (OHDSI) initiative, anchored by the Common Data Model (CDM) from the Observational Medical Outcomes Partnership (OMOP), seeks to standardize and harmonize clinical data, enabling scalable analytical tools. This report presents an architecture framework and strategy for data collection based on OMOP CDM, implemented at Hospital Israelita Albert Einstein (HIAE)/SP Brazil, useful to facilitate multicenter trials.
