Photodynamic Antimicrobial Therapy in the Treatment of Canine Otitis

Canine external otitis (CEO) is a common condition in dogs characterized by inflammation or infection of the external ear canal, which is the passage leading from the outer ear to the eardrum. It can occur due to various factors such as bacteria, fungi, parasites. Treatment for CEO often includes cleaning the ear canal to remove debris and discharge, topical antimicrobial medications such as ear drops or ointments to reduce inflammation and fight infection, and, in some cases, oral medications like antibiotics or antifungals. Due to the crescent emerge of microbial resistance the use of alternative antimicrobial methods is highly recommended. In this context antimicrobial Photodynamic Therapy (aPDT) may be an alternative to fight infections without the burden of microbial resistance selection. Patients were recruited from the Veterinary Clinic of Universidade Brasil, representing various breeds, and exhibiting clinical signs of otitis externa confirmed through clinical and optical diagnosis involving fluorescence analysis of the biofilm and ear cytology was performed. After the initial assessment, the animals were randomly assigned to 2 groups. The control group (n = 5) received appropriate antimicrobial therapy based on cytology results, with owners instructed to perform daily ear cleaning followed by drug application. The PDT group initiated with ear canal cleaning, followed by application of 5 mL methylene blue aqueous solution (200 μM), and subsequently received low-intensity red LED irradiation (640 nm ± 20 nm) for 30 s per point, totaling 5 points, delivering 7.2J to each ear. Immediately after therapy, a new cytology sample was collected, and owners were advised to continue ear cleaning for seven sub-sequent days. Follow-up appointments were scheduled at seven and thirty days after initiation, during which animals in both groups underwent further sample collection, local fluorescence evaluation, and clinical assessment. The results indicate that PDT effectively managed the signs and clinical symptoms of otitis externa.
